Teachers, Faculty, and Staff: Please complete the information on this form to request funds from the Foundation. All requests will be reviewed by the board and/or Foundation members at large at the next Foundation meeting following your request. 

Any requests over $5,000 will require a written proposal and formal presentation at a scheduled Foundation meeting and agreement by principal and pastor. You will be notified once your request has been processed. Please submit requests at least 30 days in advance.
